Free assessments in support of letters for surgery

for Transgender & Gender Non-Conforming people who are Black, Indigenous, or a Person of Color

If you are Black, Indigenous, or a Person of Color, you need a letter in support of a gender affirming surgery, and meet the criteria below, please call Shannon at (608) 520-0461 or email shannon@center4communityhealing.com and ask about the availability of a free assessment. A limited number of assessments are offered free of charge

These assessments are offered to support members of our community who do not have resources to pay for care out of pocket & do not have access to insurance networks offering care from trans competent providers

​

 

To be eligible, you will need to:

​​

• Have access to an internet connection and the ability to use our HIPAA compliant telehealth app via smartphone or computer for sessions. All of our sessions are currently remote due to COVID-19

• Live in the State of Wisconsin and be in the State of Wisconsin at the time of services per license requirements

• Be age 18 or older

Please note that an assessment is not a guarantee that a letter will be provided. You must meet the WPATH SOC to receive a letter.
